What I have done is take the blank PDF forms, print them out, scan them
in as pure black&white, and save them as .PNG files.
Then when I want to fill them out, I open with the free graphics program
that came with my scanner, "type" the info into the blanks, and use
PDF995 (a printer emulator) to create a PDF of the filled out form, and
print that out.
Of course, somewhere in there is making the picture of the
device/badge/thingy, and doing a copy&paste to add to the big blank area
on the form. ;-)
One point: I do not have a printer---well I do, but it eats $$$ of ink
even if just sitting on the shelf---so I do my printing at the local
public library, where it's free. Of course, I have to put the
PDF-to-be-printed on a floppy. If (generic) you have your own printer,
just print the filled-out form directly from your graphics program.
