For those sources that do not cite a date you need a second source.

explanation of McCoy,

In period Gaelic cultures it was vanishingly rare for a name to be 
constructed with a "middle"  (Or second given) name (Such as this 
example, Gerald Rhys McCoy).  According to what I have read Gerald mac 
Rhys mhic Aoidh would be more probable.

The other question I have is what time frame and language is each 
element from?  I'll admit to not having much in the way of books 
available at present, and Gerald is a name I do not know Time / Language on.
